Preschool Requirements        Kindergarten Requirements        First Time Enrollees in Kentucky

Students Entering 6th Grade            All Students            Graduation Requirements

Preschool Requirements:

• Certified Birth Certificate (will not accept hospital copy or wallet size copy)-within 30 days
• Social Security Number-upon enrollment
• Up-to-date Kentucky Immunization Certificate including varicella vaccine (chickenpox)
• Vision examination completed by optometrist or ophthalmologist- no later that January 1 (required for first time in public school)
• Current Physical Examination (within 1 year prior or 30 days following initial admission to school)
• Completed Federal Lunch Form

Kindergarten  Requirements:

• Certified Birth Certificate (not a hospital copy or wallet size copy)-within 30 days
• Social Security Number-upon enrollment
• Up-to-date Kentucky Immunization Certificate including second MMR and Hepatitis (HEB) B (series of 3 doses) and varicella vaccine (chickenpox).
• Vision examination completed by optometrist or ophthalmologist- no later that January 1 (required for first time in public school)
• Current Physical Examination (within 1 year prior or 30 days following initial admission to school)

First Time Enrollees in Kentucky Schools:

• Certified Birth Certificate (not a hospital copy or wallet size copy)-within 30 days
• Copy of Social Security Card-upon enrollment
• Up-to-date Kentucky Immunization Certificate - upon enrollment
• Current Physical Examination (within 1 year prior or 30 days following initial admission to school)

Students Entering 6th Grade:

• Up-to-date Kentucky Immunization Certificate- current & to include the second MMR vaccine and the Hepatitis (HEP) B (series of 3 doses)
• Second Physical Examination - within one year prior to entering 6th grade (first exam was given upon initial admission to Kentucky schools) or within 30 days of enrollment.

All Students:

• MUST HAVE AN UP-TO-DATE (CURRENT) KENTUCKY IMMUNIZATION CERTIFICATE ON FILE.

Graduation Requirements:

• Meade Co. Board of Education minimal total credits:  26
  • English :  5 Credits (inc. English I, II, III, IV, & Composition)
  • Math:  4 Credits (inc. Algebra I /Acc Alegebra H, Geometry/ Acc Geometry H, & 2 other math credits)
  • Science:  3 Credits (inc. Life Science (Biology), Physical Science (Integ Science II, Chemistry, or Physics), and Earth Science (Integ Science I))
  • Social Studies: 3 Credits (inc. Survey of Social Studies, World Civilizations, & US History)
  • Health & PE:  1 Credit
  • Computer Applications:  1 Credit
  • Survey of Humanities:  1 Credit
  • Electives: 8 Minimum
